PerMedhist stands for Personal Medical History, and was designed to help individuals maintain accurate records of their own personal medical history, while enabling medical professionals to be able to obtain an accurate and complete assessment of an individual's medical history when the individual is being seen in a hospital, doctor's office, emergency room, etc. 

     Human frailties often prevent us from remembering every little detail about our previous and/or current medical history, such as immunizations, medications, surgeries, etc.  Therefore, this handbook will serve as a useful tool to assist families with accurate record keeping of vital medical information.  Written in simple, easy-to-understand everyday language, PerMedhist allows the user to accurately record one's own medical history very easily, yet very effectively, for years to come and is ideal for every member of the family, from birth through adulthood.
